Audit and Significant Event Analysis does not have to feel like this!
The Yorkshire Deanery expect s six SEAs in each ST year and one detailed audit with completion of a second cycle during ST2 (or a reflection on a QOF performance area or a project using the Primary Care Trigger Tool). SEA
GPSTs need to be completing and uploading at least SEAs per ST year.

Primary Care Trigger Tool
The Primary Care Trigger Tool (click) was developed and tested by the NHS Institute with GP teams throughout England. The PCTT uses rapid structured casenote review to bring highly sensitive adverse event measurement within reach of every primary care team. It can be undertaken in 4-5 minutes per patient. A rapid first pass screen is performed by any member of the practice team, looking for the presence of ‘triggers’ – features in the notes known to indicate vulnerable patients, high risk care or high likelihood of adverse event. Any patient with a trigger present in the past year is reviewed in more detail by a Doctor or Advanced Nurse Practitioner, to determine if any adverse events or near miss occurred relating to the trigger. Those identified can then be reviewed as per SEA to determine cause and appropriate remedial action to be made. Repeating the PCTT will hopefully show a reducing rate of events!
In 2010 the Y&H Deanery will accept a good quality project using a number of cycles of the PCTT in place of an audit or QOF reflection. See our provisional guidance for GPSTs (click).
AuditGPSTs need to go through 2 audit cycles of an audit which is relevant to general practice, and upload it into their e-portfolio, prior to the ARCP panel at the end of ST3.
